• There are no longer any near incidents or near misses
• Either you report a "simple capsize" or "an incident" (everything else regardless of hit or miss!)
• For all incidents you will have to decide one main primary cause and one main contributory factor. Any other subsidiary causes or contributory factors must go in a text box description.
• The reason for these changes are to enable us to produce meaningful statistics
